Health Literacy Education and Awareness & COVID related Health Care Case Management
In collaboration with DWDC, Equilibrio aimed to address two critical categories in the region they served. Our main objective included a culturally competent COVID-related Health Literacy Education and Awareness Campaign. Equilibrio and the DWDC collectively possessed extensive experience in spearheading and managing health promotion efforts, including COVID-19 education, case management, and preventive health care services.
Equilibrio, established in the Washington Heights/Inwood area, specialized in culturally relevant COVID and health disparities education efforts and health literacy campaigns, focusing on improving healthcare access for vulnerable Hispanic/Latinx communities, particularly in the implementation of holistic wellness as a tool for preventive care and the treatment of long-COVID conditions. In the region served by DWDC and its partners, there was a pressing need for COVID-related interventions due to the demographic composition and socioeconomic challenges faced by the community. > The criteria for the targeted campaign were met as the region had witnessed disparities in healthcare access, health literacy, and preventive care, exacerbated by the COVID-19 pandemic.

Equilibrio and The DWDC jointly conducted a culturally competent COVID-related Health Literacy Education and Awareness campaign targeting the State's most vulnerable populations. They hired a Health Outreach Specialist for this initiative, encompassing a wide array of activities such as COVID-related marketing, public service announcements, social media engagement, virtual webinars, distribution of flyers, radio commercials, advertisements in Spanish-language newspapers, and in-person outreach during community events. Equilibrio played a consulting role, aiding in participant recruitment and delivering eight 90-minute educational workshops in Spanish throughout the year. These workshops aimed to promote the integration of clinical medicine with holistic approaches for COVID-19 and Long Covid patients, addressing cognitive impairment and various long-term symptoms.

The organization implemented a COVID-related Health Care Case Management Project, offering active case management to at least 25 vulnerable Latino residents, addressing their needs for COVID-related health services and ongoing care. The case management process encompassed intake, needs assessment, identification and removal of healthcare access barriers, service coordination, monitoring, follow-up, reassessment, case conferencing, supervision, crisis intervention, and case closure. The case manager assisted clients in achieving diverse goals related to mental and physical health, finances, immigration, substance use, COVID-19 testing access, and sexual health while ensuring they received accurate COVID-19 education. Equilibrio contributed by providing active case management support and training participants in the Integrative Health and Wellbeing program approach, emphasizing holistic and evidence-based healthcare and promoting community resource utilization for enhanced access.
